2023-24 Eastern Florida Collegiate Athletics Participation Forms

Please note: Physicals for the 2023-24 season should not be performed until July 1 or later. They may be performed by a primary care physician or other approved providers (Orlando Health Sports Medicine Group, Premier Urgent Care – Melbourne, FL, MedFast Urgent Care, or Brevard Family Walk-in – Rockledge, FL). All Student-Athletes must also submit a copy of the front and back of their Medical Insurance Cards.
